Sail Training - Participant Registration Form

COURSE REGISTRATION and MEDICAL INFORMATION FORM

For identification and safety reasons you will be required to provide personal and health information before you participate in the training program.

Join the sail training program by completing a Registration and Medical Information Form.
Fill out both pages in full and get your parent's signature.

Please bring the forms with you to your first lesson and give them to the volunteer at the Registration Desk or email to training2largsbaysc@gmail.com

WHAT TO BRING

MEMBERSHIP OF THE LBSC

For insurance purposes, all sailors participating in Sail Training Programs shall be members of LBSC.

We require that your child become a club member after 1 sessions, if they wish to continue to participate in the training program. Please complete a membership form and pay the "Youth Sailing" fee.
